Supraspinatus Tendinitis
Supraspinatus Tendinitis is also referred to as Rotator Cuff Tendonitis, Swimmer's Shoulder, Pitcher's Shoulder or Tennis Shoulder.
There are 2 types of rotator cuff tendonitis - Acute and Chronic:
- Acute tendonitis refers to inflammation that comes on suddenly, usually from a shoulder injury, such as a fall causing dislocation (typically a person may even develop Frozen Shoulder), overloading it during exercise, or lifting something too heavy overhead.
- Chronic tendonitis develops over-time and generally results from long term repetitive use of one or some of the rotator cuff tendons. Common activities that cause chronic tendinitis in the rotor cuff are activities like weight lifting, painting, and repetitive throwing in sports. With both types of tendinitis, the condition can be very painful. Likely you have periods where it flares up, and periods when it feels somewhat better after it has been rested or the repetitive motion that irritates it has been stopped.

Tendons
Tendons are bands of connective tissue that join your muscles to your bones. This connection allows you to move your body when you contract your muscles. The rotator cuff (also called the rotor cuff) is made up of 4 tendons located at the outside edge of your shoulder that form a "cuff" around the glenohumeral joint. These tendons, along with other soft tissue in the joint cavity, work together to attach the humerus (upper arm bone) to the scapula. When the shoulder muscles contract, these tendons; the supraspinatus, the infraspinatus, the teres minor and the subscapularis, allow your shoulder to complete its complex set of movements and move your arm.

Tendonitis
Tendonitis (also spelled tendinitis) is inflammation in a tendon. When you are diagnosed with rotator cuff tendonitis, it means that one or more of the four tendons in the rotator cuff has become inflamed. The inflammation of tendonitis is usually a result of tiny tears or repetitive irritation of the tendon.
Although tendinitis can occur in any of the rotator cuff tendons, the supraspinatus tendon is the tendon most at risk of injury. Due to its location at the top of the shoulder - between the joint cavity and the rigid bony arch of the shoulder blade (acromion) - the tendon is at risk from irritation, wear and tear, and/or the nerve becoming trapped in the soft tissues of the shoulder (impingement). The attachment of the muscles and tendons to the acromion provides the leverage that helps to lift the weight of the arm away from the body (abduct). As most of use use our arm for many daily activities, it is a fairly common injury; if this tendon injury is ignored, it will likely worsen and can lead to chronic (long term) tendinitis.
Symptoms of Rotator Cuff Tendonitis
The symptoms of chronic rotator cuff tendinitis usually begin with mild pain in the shoulder that gradually becomes worse. Acute symptoms will come on more suddenly. If you have rotator cuff tendinitis, some possible symptoms may be:
- Pain in the top and front of your shoulder that becomes worse with overhead activity. Initially, the pain is felt during activities only but eventually you will feel it even while you rest your shoulder.
- Limited range of motion in your shoulder's glenohumeral joint.
- Tenderness and a burning sensation in your shoulder.
- The feeling that the small muscles in the shoulder do not make small adjustments smoothly, or pain that results from these small motions.
- Difficulty sleeping at night due to pain, especially when lying on the affected shoulder.
- Difficulty with simple movements such as brushing hair, putting on your shirt or jacket, reaching the arm above shoulder height.
- A feeling of weakness in the shoulder, especially with pushing and overhead movements.
If you've suffered a traumatic sudden injury to your shoulder, you may have heard a sudden popping sound or felt a tearing sensation in your shoulder.
Swelling around your shoulder will happen within a few hours after the injury or in the following days. If the injury is severe enough, you may also have some bruising on your shoulder. Immediate, intense, pain will follow - especially when you try to raise, lower, or rotate your arm.
If your rotator cuff tear is big enough you might feel weakness (instability) in your shoulder. Over time this weakness will turn into a limited range of motion and inability to raise, lower, or rotate your arm - resulting in pain. At this point your rotator cuff injury would also be interfering with your sleep, causing pain and night and forcing you to wake up several times to adjust your sleeping position.
Degenerative rotator cuff tears are subtle with pain that increases over time.
You may also notice swelling after exercise or daily activities. If you do have pain, you may experience something called 'trigger point pain' in your shoulder and possibly feel weakness / instability. Trigger point pain is pain that comes from a specific spot in your rotator cuff muscle that has become a hypersensitive area. These trigger points, or hypersensitive areas of pain, are known to be a source of on-going pain and lack of blood flow within the tissue. Pain may increase with certain activities, like raising your arm to reach for something off of a high shelf, brushing your hair or teeth, getting dressed, or reaching behind your back.
Causes of Rotator Cuff Tendonitis
- A hooked acromion at birth increases your risk of rotator cuff tendinitis and bursitis as they can become impinged in the subacromial space.
- Slouching forward can narrow the space your tendons have to move in the shoulder creating excess pressure on them. This can irritate the tendons and/or interrupt the blood supply resulting in breakdown of the tissue.
- Repetitive overhead motion such as serving a tennis ball, pitching a ball, hammering, or painting above the head.
- Lifting heavy objects or free weights above shoulder height.
- Aging; as we get older, our tendons become more brittle and therefore more prone to injury.
- A weak rotator cuff can cause misalignment, putting unequal stress on the tendons.
- Shoulder injuries or having associated conditions like arthritis, diabetes, and thyroid disease.
How Do I Diagnose a Rotator Cuff Tear?
When you are experiencing one or more of the symptoms listed above and suspect you have this condition, the best way to diagnose it is with a quick visit to your doctor for a physical examination of your shoulders, neck and back. Getting a proper diagnosis is important so you can treat your condition correctly. An untreated tendon tear in the rotator cuff will likely lead to a worsening of symptoms and will continue to freeze up the range of motion in your shoulder.
To begin with, your doctor will gather a medical history about you and your current condition and symptoms. He/she will ask about the amount pain you are having with your shoulders and back; questions related to the duration of symptoms and range of motion loss should be expected. Further details about what you suspect initiated the issue, when it started, and whether or not you have ever had treatments for this or a similar condition in the past are very helpful in assessing your injury. Your doctor may also consider any previous back, shoulder and neck injuries or joint stiffness that you may have had in the past. This will help the doctor to determine if you have a more complex injury in the area and rule out any other injuries that may have once been symptomatic.
Range of motion movements / tests will be done to see how much movement has been lost in your shoulder and upper arm. You may experience minor temporary pain from the doctor poking and prodding the area, but by doing this, your physician will better understand the source of your pain, the level of tenderness in the area and how well you can move with this condition.
If a soft tissue injury is suspected, an MRI may be undertaken to view where and how extensive the damage is. An x-ray may be recommended to rule out other possible causes of your symptoms. Possible causes of pain that an x-ray can identify include instability, abnormal bone shapes (bone spurs or bone cysts), wear and tear on the joints, avulsion fractures (when a fragment of bone tears away from the main bone along with the muscle, tendon, or ligament as a result of trauma), dislocations, arthritis, calcium deposits in the muscle or joint, and/or other problems.
Rotator Cuff Tears and Strains Do Not Have to Persist For Years Or Longer. Here is Why It Does For Many
Recovery from a strained or torn tendon can take a long time, especially when adequate rest is not taken and some of the less effective, traditional treatment methods are utilized. This is simply because many treatments either focus on masking of pain or are too inconsistent to provide sustainable improvement.
1: Rotator Cuff Injury Sufferers Often Aggravate & Re-Injure Their Shoulder
It goes without saying that anyone suffering from a shoulder injury should want to minimize their chance of worsening the injury, or re-injuring it once it has started to get better. Obviously, a worsening injury will delay the healing process, but what's worse is that every re-injury and additional healing cycle increases the amount of time that your joint is immobilized. Time is not kind to an immobilized joint - you lose strength in the joint but worse than that, you lose the range of motion in your joint via atrophy (your flexible tendons, muscles and ligaments slowly shrink, decreasing joint elasticity). The more time that goes by with an immobile shoulder, the more likely you will wind up with a chronic problem. Basically it means that your shoulder will not perform as well as it once did and it becomes more prone to injury again later on.
Minimizing the time spent with an immobile shoulder joint should be an obvious goal, as a rotator tear will typically limit your ability to go about your daily routine.
2: Visits to Physio (PT) Help - We Always Recommend It - But It Can Be Costly And Inconvenient
While we tout the benefits of PT and believe it to be integral for recovery from soft tissue injuries, the problem is that people only attend sessions a few times a week. To truly provide a long term solution to combat atrophy and an inactive shoulder joint, you need to be receiving multiple treatments on a daily basis... Not just any type of treatments, but ones that focus on strengthening and improving tissue flexibility in the joint.
Treatments for soft tissue injuries are cumulative in nature...meaning the more the treatments are administered, the faster the benefits can be experienced. Who these days has time to visit a PT on a daily basis? Not only is your time limited, but the cost of daily clinic visits are out of reach for most people.
Therefore, the focus needs to be on adding treatment options that can be done frequently, from the comfort of your own home (and even the workplace if the opportunity presents itself).
3: Injections Can Cause Harm If You Are Not Careful
This can be VERY helpful in alleviating pain and in many cases, pain relief alone can make a huge difference. However, some people get a false sense that their injury is healed because they are not experiencing the pain they once did. Some individuals even engage in activities they should not be doing because they "no longer feel pain". Once the effects wear off, people often find their troubles persist . This of course, results in a prolonged recovery. Be very careful about your activity after the injection because there is little, if any, pain feedback to tell you that you're overdoing it.
Merely masking the pain can be very helpful temporarily, but it is not a solution; it is just a temporary fix.
4: People Tend To Focus Too Much On Cold Treatments
We see it all the time - people for weeks and even months only focus on using cold.
Doing too much cold will actually inhibit the recovery because you are starving the injured tissues of the blood flow they need to repair and rebuild the collagen. Cold restricts the vessels - reducing the blood flow to the tissues that truly need it most.
Cold treatments should be more focused on in the earliest stages of an injury with much less focus in the mid and latter stages.
The truth is, there will always some mild inflammation associated with tissue injuries. However, some of that inflammation will remain UNTIL you begin to focus on healing the soft tissue (muscle). Once this begins to heal, the inflammation will then subside more permanently - not just temporarily like with cold treatments.
As an analogy, think of a home with a leaky roof. You can place a bucket on the floor of the attic whenever it rains - and this will help temporarily, but does it truly address the situation? Obviously the answer is no. Not until you focus on repairing the structural issues with the roof - will the problem be actually solved.
The same concept applies to injured soft tissue - you need to focus on reinforcing growth of flexible, strong tissue. To do this, you need more blood flow reaching them, not less.
Rotator Cuff Tendonitis Home Treatments
If you have a rotator cuff tendinitis, rest is highly recommended. Avoid activities that cause pain, aggravate the injury, or may have caused the inflammation. Begin cold compression treatments as soon as possible.
Treating your rotator tendonitis is essential to getting rid of pain and restoring function to your upper arm and shoulder. Proper treatment will get you back to regular activities sooner, stop your pain, and reduce the risk of future re-injury.
Your doctor will assess your particular case and determine a course of action. In most cases, a conservative treatment protocol will be enough to heal the injury, though in cases such as significant tearing or a fully ruptured quad, you will most probably require surgery. It is generally understood by doctors and surgeons, that surgery will introduce more scar tissue into the any already damaged tissue. This added scar tissue will be problematic, requiring more physical therapy and conservative treatment options post-surgery. If not dealt with properly, your muscle injury could end up in worse condition than before the surgery! This is why surgery is only performed as a last resort.
To restore strength and range of motion in your rotator cuff, treatment should focus on minimizing further damage and preventing both scar tissue formation and muscle atrophy (shrinkage and weakening of the shoulder muscles). This requires rest and the appropriate therapies at the right time. Almost all types of pulls and tears (except a complete rupture) can be well treated with trusted options that are available for use at home. Complete ruptures require surgery - however, using these home treatments after surgery (once your surgeon/physician approves) can help speed recovery, improve function, and increase range of motion in your shoulder, arm and back.

Not only will you use your cold pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...

- If you have a job that requires you to place some demands on your strained rotator. If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
- If you are in a post-operative situation where cold & compression would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming surgery, then you will need a high quality cold pack to use.
